I am aware of the current Public Health Order requiring proof of vaccination to participate in Community Sport.

I understand my obligation as a Hills Hornets Touch representative in the role(s) identified above requires me to only attend competition, training and other face-to-face club activities if able to meet the double vaccination status.

I understand that should I fail to adhere to this requirement, my association with Hills Hornets Touch will be subject to disciplinary action imposed by the Club and/or State body.

I acknowledge the Public Health Order obligations stated here, and agree comply.
